Prof.Tomaselli: Middle age needs to be looked out in the context of what other risk factors are present. Obviously the key thing internationally as well as nationally is tobacco use. Tobacco use is still a significant problem in this country and even bigger problem globally. But again, an isolated hypercholesterolemia in the absence of elevation of blood pressure and the absence of other risk factors, you might choose to treat that with a combination of diet and physical activity, assuming it is not astronomically high. If blood cholesterol remains high and if you are going to treat with a statin because there are other risk factors, you really need to treat and treat in a way that drops the LDL-c <100, perhaps <70. When it has been looked at, there really is a linear correlation between the event rate and the level of LDL-c.
Tomaselli教授:中年人还应当关注存在其他哪些危险因素。很明显,不管是在国际上还是美国国内,最重要的危险因素都是吸烟。吸烟在美国仍然是个很大的问题,在全球范围内更是如此。但是,如果只是单纯的高胆固醇血症,胆固醇水平不是特别高,而且没有高血压以及其他危险因素的话,还是应当进行饮食控制和加强体力活动,如果胆固醇水平仍然升高,或者因为有其他危险因素而接受他汀治疗的话,需要把LDL-C降到100mg/dL,或者70mg/dL以下。因为根据以往的研究,事件率和LDL水平呈线性相关。
